The goal of OCHN is to ensure that these individuals have access to resources that improve their health and quality of life, as well as engage them in community participation. OCHN provides over 300 services at 27,000 sites across the county and is insured by Medicaid. Further information about programs and supports provided by OCHN can be found at www.oaklandchn.org.
